Parents and/or Guardians of Students Representing School No. 7 and No. 7-70 in all school activities or involved with School Transportation.

Recently, it has become exceedingly difficult to obtain Medical Services for students injured without first obtaining parent/guardian consent in writing. So that proper emergency assistance may be provided, we ask that you review the following statement. It is also understood that this form will be valid as long as your child is enrolled in Laurel Public Schools, and any medical expenses will be paid by parents/guardians or their insurance company.

So that proper emergency assistance may be provided, I hereby authorize Laurel Public Schools and their personnel in charge of the child named above to obtain all necessary medical care, and I hereby authorize any licensed physician and or medical personnel to render necessary medical treatment.
